Annual Athletic Physical Form (Part A and Part B)

NJ State regulations require student’s participating in middle school intramurals and/or sport to submit two physical forms, one completed by the parent (part A) and one completed by the student’s physician (part B). A new part A is required prior to each sport season. Part B is valid for one year from the student’s physical date and this date must fall before the first day of participation. Otherwise, a new part B is required.

INTRAMURALS - Mr. K.H. Lecour, Director

The Intramural program is aimed towards an enjoyable means to experience athletic team competitions without the high level of pressure often associated with other in-school or in-town athletic activities.

Attractive medals and certificates will be awarded to members of series winning teams. Everyone plays an equal amount of time. Teams are selected in a manner that prevents any possible embarrassment caused by the order of selection of team members. There are no tryouts or cuts.

The program is flexible enough to encourage student participation in other school activities at Annin. Intramurals also enables middle school students to unwind after a very arduous academic day. Experienced and very enthusiastic William Annin faculty members supervise the Intramural Program. 

Late buses are provided at 3:30 for students who have a bus subscription.

Winter Session Intramurals

The Winter Session will begin in December and continue through the month of February. If you have already paid the annual activity fee, then it will not be necessary to submit the Winter activity fee of $100.00, and to complete and submit the permission slip attached to the bottom of this notice.

Activities that will be featured during the Winter Session are listed below.

Featured Activities:

  • Blue and Gold Volleyball (December)
  • Tournament Volleyball (December)
  • Table Tennis Tournament (January and February)
  • Basketball (January and February)
  • Floor Hockey (January and February)

Schedules:

Schedules will be made available prior to the Winter Session. The Intramural Program takes place from 2:25 until 3:25 on Mondays, Tuesdays, and Thursdays.

Invitation:

We encourage your son and/or daughter’s participation in William Annin’s Intramural Program. We provide a healthy level of competition, promote a positive interaction between participants, and enable a means to unwind after a rigorous academic session at Annin. Win or lose, the real emphasis in the Intramural Program is to provide an enjoyable and rewarding experience for all the participants.

Permission Slip, Activity Fee, and Medical Forms

Note: It is not necessary to submit the activity fee or permission slip, if you have already submitted the $235.00 annual fee/permission slip prior to or during the Fall Session. Click HERE for permission slip.

State regulations now require that Intramural participants between grades 6-12 must complete medical forms, one by the parent(s) of the participant and one by the participant’s physician. If the winter session is the FIRST SESSION in which your child will participate this school year, then it will be necessary to complete Form A and Form B of the physical forms prior to your child’s participation. The physical must have been completed within a year of the date of the beginning of your son or daughter’s participation. Annual participants who have already submitted Part A and Part B will not have to submit these forms again.
