If you had a physical in the last 365 days from the first day of tryouts and it is on file with the Ridge High School Athletic Office, then please complete Packet A (permission, contract) AND pages 1-3 (parent completed) of Packet B AND the Cognitive Testing Release Form. If your physical is older than 365 days from the first day of tryouts or is not on file with the Ridge High School Athletic Office, please complete Packet A (permission, contract) AND pages 1-7 (parent and doctor completed) of Packet B AND the Cognitive Testing Release Form. Optional Online Physical Version New in-school athletic physicals are here!! Cognitive Testing will be performed by our Athletic Trainers. The signed permission form will be valid for your student’s four years at Ridge High School. Testing is done in the freshman and junior years. For further information on Cognitive Testing, please visit www.impacttest.com. NJSIAA Steroid Testing Consent must be completed only once per school year. Please read the Steroid Cover and Banned Drug Classes. Print and complete the Policy Consent and return prior to your first sport participation.
